Referalls

Where possible we aim to ensure a positive planned transition from previous placements or birth family homes in order to work closely with individuals to ensure a smooth transition. We will facilitate transitions from previous residential homes, secure centres, foster care and family homes and will aim to secure as much detail as possible in regards to the child’s individual needs in regards to likes, dislikes, behaviour management strategies and any current risks.

We will match the children with our other current or planned young people within our homes as well as possible and in accordance with our statement of purpose or location community impact assessment.

Emergency placements will only be considered if the child’s known behavioural patterns and contact arrangements are accessible prior to the child being placed. We will require an individual placement request document where identified risks are listed and we will then ensure that the child or young person can be kept safe within the home local community if offered accommodation with us.
